The image is quite an old set from Inkadinkado the sentiment and hearts are also in the set. I made my own card blank using Spellbinders Grand Scalloped Rectangles and Dienamics Blueprints. The papers are from Sweet Shoppe Designs "Measured in Love". I used my Lil' Inker Stitched Pennant for the sentiment. Here is a side view:
